From routine maintenance to immediate response: Here’s how to contact Facilities

Whether you have urgent facilities issues or need routine maintenance services, it’s important to know the right way to contact the Facilities team. Here are some common services Facilities offers and how you can easily request them: 

Work Requests – These are non-emergency, general maintenance and repair requests. Examples include: leaking faucet, burned-out light, damaged wall or ceiling and hanging a picture. All routine work requests can be entered into the computerized maintenance management software where it will be evaluated and assigned to a maintenance professional. You can initiate a work request through the Infonet.

  • Click on Facilities in the Shared Services column on the Infonet homepage.
  • Click on the Work Requests
  • Select your location to start the work request form.

You will be updated by email as your work request is addressed and progresses.

Immediate Response – Contact Facilities immediately to report any type of utility failure or potential hazard, including but not limited to: water incursion; electrical failure or hazard; and potential for personal injury. You can contact your location’s Facilities Department by calling the phone number listed under your location’s homepage on the Infonet. Call your site’s emergency number or 911 for critical emergencies such as fire and follow your department’s emergency plan.

Request an Estimate (RFE) – An RFE form is used to request a cost opinion for a renovation or reconfiguration of an existing space. Examples include: Modifying an open office into individual offices and changing the finishes in an office or exam room. You can access the RFE form through the Infonet.

  • Click on Facilities in the Shared Services column on the Infonet homepage.
  • Click on the Request an Estimate
  • Complete the PDF form, scan and submit it to the email address indicated.

Please note that the RFE must be signed by a department chair or vice president in order to be processed. Once a completed form is received, a facilities project manager will contact you with next steps.

Space Request – This is a request for more space for your department or to change the use or occupancy classification of a space you occupy. In some cases, changing the use of a room will require regulatory approval. Typical reasons for submitting a space request include: A new employee needs a workspace; your department or service is expanding; or you need to repurpose an underutilized space. You can initiate a space request through the Infonet.

  • Click on Facilities in the Shared Services column on the Infonet homepage.
  • Click on the Space Request
  • Complete the online form and click submit.

Space requests must be supported by a department chair, vice president or chief nursing officer and are evaluated by a multidisciplinary team on a monthly basis. A facilities staff member may reach out to you for additional information or inform you of the status of your request.

Additional Services ─ Facilities provides many additional services including:

  • Creating signage and posters
  • Making keys
  • Identifying safety concerns
  • Installing temperature and environmental monitoring sensors
