Care Transitions welcomes seven to Clinical Ladder

Care Transitions at Hershey Medical Center inducted seven team members into its Care Transitions Professional Development Clinical Ladder II, a program it implemented in 2019.

Inductees were Inpatient Social Worker Danielle Rummel, Social Worker Meghan Bragers, Inpatient Social Worker Carol Moore, Inpatient Social Worker Gina Klein, Clinical Case Manager Karen Wilson, Outpatient Social Worker Kristin Vines and Register Nurse Outpatient Case Manager Dawn Barrick.

Each of the registered nurse care coordinators, managers and social workers who were named demonstrate a high level of professional engagement in their daily work, reinforced by high-quality documentation.

“Ongoing professional clinical development and contribution increase the caliber of our practice and ultimately improves patient outcomes and experience,” said Care Transitions Vice President Monica Cascarino.
