Information Services to launch electronic account request form

On June 17, Information Services will launch an electronic account request form that will automate much of the manual process currently associated with requesting accounts. The new form and process will eliminate the need for printing, signing, scanning and emailing. Managers will approve or reject an account request directly from their email.

To ensure a smooth transition, the paper account request form will remain available and will be accepted for six weeks, until July 31. Beginning Aug. 1, only eARFs will be accepted.

The eARF will be available on the Account Management Infonet site, and the quick reference guide and job aid will support adoption and use of the eARF. The eARF and other IS forms are also available through the IS Service Portal.
